Since April 1, 2005
Based upon the current MIDWEST AVERAGE Diesel Fuel Index,
QS Delivery Service Inc., fuel surcharge is now available.
The Table listed below shows the applicable amount based on
the EIA retail on-highway MIDWEST AVERAGE Diesel Price.
The level of the fuel surcharge is adjusted every Monday
based upon the MIDWEST AVERAGE Diesel Fuel Index. This average,
published by the Energy Information Administration of the
U.S Department of Energy, is available by telephone at
202-586-6966
and on their website at:
